בהתאם לתיקון 13 לחוק הגנת הפרטיות, כל אדם רשאי לעיין במידע שנאסף עליו,

לבקש את תיקונו או מחיקתו, או להתנגד לשימוש במידע לצרכים שיווקיים.
טופס זה מיועד להגיש בקשה רשמית למימוש זכויות אלה.

• הבקשה נועדה למידע אישי שנאסף באתרי mokile.net ו-monicadesign.net בלבד.

• ייתכן שתידרש הזדהות באמצעות כתובת הדוא"ל שלך כדי לאמת את זהות מבקש/ת המידע.

• נענה לכל בקשה תוך 30 יום ממועד קבלתה, בהתאם לחוק.

• ייתכן שנשמור עותק של המידע ככל שנדרש לצרכים חוקיים (חשבוניות, תיעוד עסקאות וכו').
